Choose the type of Membership you want by clicking on the "Add to Cart" button next to the type. (Choose ONLY ONE type, please) This will take you to the secure PayPal site where you can use a credit card to pay your annual Membership dues. If you need an official Invoice or Request for Payment in order to pay your annual fees, UPHA's and PayPal will send you one of these documents. Choose which of these you need from the two choices below, enter your email address, click on the type of UPHA Membership you will be paying for, then click on the "Submit" button. Within a few days, the document you request will be sent to the email address entered.
