How to apply
New members
Visit the membership application system to apply for new membership and pay dues. Alternatively, print the paper application form and mail to the address on the form. Dues must accompany the application and checks must be payable to Utah Public Health Association.
Current renewing members
You will be notified by email or letter with your personal login and password to access the membership application system. Upon receipt of this notification, you will be able to log in, update your information and pay dues.
