Membership - UPHA is the largest association in Utah dedicated to expanding public health. With over 200 members statewide, members enjoy the benefits of reduced conference fees, a newsletter, membership on the UPHA email List, leadership opportunities and many other benefits. Annual Renewal Process - To assist in renewing Membership in the association, UPHA sends out a personal letter and Renewal form at the beginning of each year. The form contains information that you have provided us in the past. Please review the form, making sure ALL information is current, and return it to the address shown. You can pay you renewal fees with your check or money order sent with your Membership Renewal form, or see below for our new  option.
If you have not received the Renewal form or want another, click HERE . Then, send a request for your personalized Renewal form to the UPHA Membership Chair and we will send you another. (We send the forms to your address we have in our member database, so if you have a new address please provide this to us in your message.)
